Interventions for Health Promotion and Obesity Prevention for Children and Adolescents with Developmental Disabilities: a Systematic Review

30 août 2022

Rev J Autism Dev Disord. 2022 Aug 22:1-24. doi: 10.1007/s40489-022-00335-5. Online ahead of print.

ABSTRACT

This systematic review evaluated interventions and relevant outcomes for health promotion and obesity prevention among children and adolescents with developmental disabilities (DD). Twenty-one studies including randomized control trials (n= 9) and quasi-experimental studies (n=12) published between 2010 and 2021 met inclusion criteria related to participant characteristics, intervention type, and child obesity-related outcomes. Five types of intervention programs were identified: aerobic and strength training, sport-based physical activity, aquatic exercise, active video gaming, and diet and lifestyle. Whereas analysis of intervention outcomes, efficacy, and study rigor showed mixed results and weak evidence of effective interventions, this review identified gaps in the literature, promising strategies for addressing obesity in children with DD, and implications for practice and future research.

The Twenty Five Most Important Studies in Health Promotion

23 août 2022
American Journal of Health Promotion, Ahead of Print.
Which are the studies, special reports and commentaries that have been most influential in shaping the health promotion profession? This editorial poses that question to many of America’s most accomplished researchers. Each was asked to name one or two ‘must read’ studies from other scholars as well as to feature one of their own research projects that has had the greatest reach. This review of seminal studies focuses on community health, patient education and behavior change research and a future editorial will focus on workplace based health promotion research. Readers are challenged to review the four decades of research represented by this list and consider whether trends can be identified with respect to the relative attention researchers are giving to individual, interpersonal, community and societal factors influencing health behavior. How clear is the evidence that the choices we make are determined by the choices we have?

Impact of the use of Red Lotus Critical Health Promotion Model as a pedagogical framework on health promotion graduates' professional practice: a mixed methods study

11 août 2022

Health Promot J Austr. 2022 Jul 25. doi: 10.1002/hpja.642. Online ahead of print.

ABSTRACT

ISSUE ADDRESSED: The Red Lotus Critical Health Promotion Model (RLCHPM) is designed to support critical health promotion practice. This study investigated the impact of the use of the RLCHPM as a pedagogical framework for competency-based university curricula on the practice of graduates from health promotion programs from an Australian regional university.

METHODS: A mixed methods study was undertaken, including an online survey of all 195 graduates from 2008 to 2016, followed by semi-structured interviews with a subset of respondents.

RESULTS: There were 95 survey respondents and 10 interviewees. More than half reported the model impacted health promotion programs they are involved in, however less than a quarter felt it impacted workplace policies. Impact was significantly higher for those with higher levels of knowledge about, confidence in using, and perception of utility of the RLCHPM, and stronger alignment of their practice with critical health promotion values and principles. Graduates' embodiment of the model's values and principles in practice enhanced the impact of the model. Factors that limited the impact included participants' implicit use of components of the model without explicit reference to the model, and the incongruence between participants' professional practice ideals and those of their workplace context.

CONCLUSIONS: The use of the RLCHPM as a pedagogical framework for university health promotion programs positively impacted on graduates' practice within the Australian context. SO WHAT?: The RLCHPM could be used as a pedagogical framework in universities to develop competency-based critical health promotion curricula to enable graduates to progress critical health promotion practice.

Volcano: Between Structural Vulnerabilities and Collective Defence of Honor in Communities Surrounding an Open Dumpsite

27 juillet 2022
Qualitative Health Research, Ahead of Print.
This study analyzes the dynamics in which structural vulnerabilities are shaped and expressed in the day-to-day lives of people making a living from a garbage dump in Argentina. It is a mixed methods case study, qualitative driven with a collaborative cycle (2016-2021). The study is embedded in medical anthropology, social epidemiology and Latin America Social Medicine, with the focus on the ways people respond to inequalities locally. The findings contribute to the field of health disparities in three directions: 1) the description of patterns of segregation in South America peripheries, shedding light on cumulative disadvantages and multiple exposures; 2) the experience of living enmeshed in places with strong stigmas, revising the ways this source of discrimination become social suffering; and 3) the detection of the impact of collective action and social capital in providing material and symbolic resources for restoring dignity and honour that challenge depreciated status.

Caring for people living with dementia in their own homes: A qualitative study exploring the role and experiences of registered nurses within a district nursing service in the UK

20 juillet 2022

Int J Older People Nurs. 2022 Jul 18:e12491. doi: 10.1111/opn.12491. Online ahead of print.

ABSTRACT

BACKGROUND: In the UK, district nursing services (DNS) deliver care to people intheir own homes and have regular contact with people with dementia. Research conducted with nurses working in similar roles outside the UK suggests their contribution to high quality dementia care is limited by compassion fatigue, lack of dementia training and low levels of confidence. However, there is a paucity of research exploring the role and learning and support needs of nurses within DNS.

OBJECTIVES: The aim was to gain insight into the role and experiences of nurses caring for people living with dementia at home.

METHODS: The study was informed by a descriptive phenomenological approach. Semi-structured interviews were conducted with a purposive sample of ten nurses working in DNS. Data were analysed thematically.

RESULTS: Five main themes were identified: 'Home as a care setting' reflected how delivering home-based care shaped participants experiences of caring for people with dementia; 'Taking it in their stride' revealed how participants adapted and responded to the complexity of care needs for people with dementia; 'Complexity and unpredictability' related to the unpredictable nature of people with dementia's care needs and the impact this had on participants' workloads; 'Expertise and support within the wider team' detailed which networks nurses used for advice and support to manage the complex needs of people living with dementia at home; 'Specialist support' identified the need for structural changes and resources to enable the nurses to deliver the care needed.

CONCLUSIONS: This study enables better understanding of the role of DNS in supporting people with dementia to live at home. This is important for defining how dementia care can become effectively integrated into primary care. Recommendations include improved models of care, which factor in specialist nurses, additional time for home visits and greater emphasis on education and training.

IMPLICATIONS FOR PRACTICE: Improved models of working that factor in additional time and staffing such as specialist nurses in dementia and palliative care would allow DNS to meet the needs of people with dementia more effectively.

How can participation in health promotion projects be encouraged ? Professionals' views on the key factors at play.

20 juin 2022

Rev Epidemiol Sante Publique. 2022 Jun 14:S0398-7620(22)00362-5. doi: 10.1016/j.respe.2022.05.005. Online ahead of print.

ABSTRACT

INTRODUCTION: The participation of the stakeholders concerned by health promotion interventions targeting health determinants is a founding principle of integral importance. Notwithstanding the potential benefits of these approaches, their implementation is still quite limited and field practices are highly varied, if not totally heterogeneous. Such limitations can be considered in conjunction with the poorly defined outlines of participation, which can lead to variable interpretations of what it entails, and also to the different contextual factors potentially affecting its implementation. This study therefore aims: (1) to identify the various perceptions and experiences of participation, and (2) to identify the factors impacting the implementation of participation in support and development of health promotion.

METHODOLOGY: All in all, 34 professionals participated in this qualitative research, in which triangulation was associated with analysis of the written productions collected during creativity workshops (Cube activity) and semi-structured interviews and observations. All participants gave their informed consent to participate, and the data were anonymized and remained accessible to one and all. The data were subjected to content analysis (Bardin, 2003) focusing on types of factors contributing to the implementation (or non-implementation) the of participation.

RESULTS: Data analysis led to the emergence of 7 categories of factors: stakeholder characteristics, the individual characteristics of professionals and decision-makers, relations between professionals among themselves and with stakeholders, the methods and form of the approach implemented, the local organization and its missions, and the national context.

DISCUSSION: The representations and experiences of participatory approaches among health promoters are very heterogeneous. Implementation depends largely on how stakeholder characteristics are taken into account, on the ability of professionals to adapt to them, on the development of favorable interpersonal relationships through reflexive work on posture, and on the use of relevant and scientifically validated methods.

CONCLUSIONS: To strengthen the openness and motivation of field professionals to undertake participatory actions, awareness-raising and training in specific skills seems relevant, the objective being to enable them to anticipate risks and to make the most of whatever opportunities appear.

Managing Uncertainty for and With Family: Communication Strategies and Motivations in Familial Uncertainty Management for Hereditary Cancer

14 juin 2022
Qualitative Health Research, Ahead of Print.
The management of uncertainty is integral to health and illness. Individuals manage uncertainty about their health through communication enmeshed in family systems, but existing theorizing focuses on individuals without accounting for family processes. An iterative qualitative analysis of 42 dyadic, family interviews (N = 84) revealed (a) moments in the context of hereditary cancer that involved individual-centered and familial uncertainty appraisal and management, (b) family members’ communication strategies to prompt relatives to engage familial uncertainty, and (c) the communicative (re)creation and negotiation of family models for uncertainty management. The findings illuminate tensions that individuals encounter across their lifespan as they appraise and manage uncertainty about hereditary cancer risks. This study extends uncertainty management theory to encompass familial uncertainty management and contributes insights useful for the management of hereditary cancer.

Barriers to health promotion with overweight or obese children, young people and their families

31 mai 2022

Nurs Child Young People. 2022 May 23. doi: 10.7748/ncyp.2022.e1429. Online ahead of print.

ABSTRACT

Childhood obesity is a worldwide public health issue requiring sustained health promotion efforts by multiple healthcare professionals. Children's nurses are the main providers of education, support and interventions for those who are overweight or obese. Despite sustained health promotion efforts, addressing overweight and obesity in children and young people is an ongoing challenge. This article details a literature review that aimed to identify the barriers that children's nurses experience when undertaking health promotion to address overweight or obesity with children, young people and their families. Three themes were identified: lack of time, training and resources; parental motivation and response; and the 'new normal'. Education and work-based training on structured interventions for addressing overweight or obesity are vital. Skills training in broaching challenging conversations with children, young people and their families sensitively and effectively is also important.
