This paper has been in preparation for too many years and we have had comments from too many people to thank them individually. This should not be taken as a sign of lack of gratitude. However, we would like to recognize our great debt to Louis-André Gérard-Varet, with whom we began this project, and with whom it would have been such a pleasure to keep working. We have prepared an “extended” version of this paper, which includes some supplementary material, especially details of computations which were too long to be included in the “official” version.
